Over the last decade, Senior Doc has been providing personalized care management services to meet the individual needs of each patient and family in their healthcare journey. Being accepted into CMS’s GUIDE program will enable us to broaden our impact and reach further.


GUIDE Support and Services

Care Management

An interdisciplinary team develops a customized care plan that includes ongoing monitoring and regular updates.

Care Navigation

A committed Care Navigator collaborates with the clinical team to manage communication, appointments, and medication, as well as make referrals to community services.

24/7 Support

Beneficiaries and caregivers will be able to reach a member of their care team or navigator through a 24/7 helpline for assistance.

Caregiver Education

A program for caregivers that offers training, education, and support, with Care Navigators available to help coordinate access to both clinical and non-clinical resources.

Respite Services

Caregivers could qualify for respite care services, with an annual benefit of up to $2,500.


Instant access to a wide range of educational classes, content, and support groups covering various topics related to caring for individuals with dementia.

Training

Medication Management

The clinical team frequently evaluates prescribed medication as necessary, with care navigators offering guidance on effectively following medication schedules.

  • Have a dementia diagnosis

  • Enrolled in Medicare parts A&B (not Medicare Advantage or PACE)

  • Not enrolled in Medicare hospice benefits

  • Does not reside in a long-term nursing home (assisted living facilities accepted)

  • Have Medicare as their primary payer
