Senior Doc is excited to announce its acceptance into the CMS’s innovative GUIDE Model. The program is designed to provide additional support and resources for individuals living with dementia.

Over the last decade, Senior Doc has been providing personalized care management services to meet the individual needs of each patient and family in their healthcare journey. Being accepted into CMS’s GUIDE program will enable us to broaden our impact and reach further.

GUIDE Support and Services

Care Coordination & Management

Interdisciplinary team creates a personalized care plan with continued monitoring and updates.

Caregiver Support & Education

A training, education, and support group for caregivers. Care Navigators coordinate access to clinical and non-clinical resources.

Respite Services

Caregivers may be eligible for respite care services, up to $2,500 annually.

Navigation

A dedicated Care Navigator coordinates communication, appointments, & medication in partnership with the clinical team. Referrals to community services.

24/7 Support

Beneficiaries and caregivers will have access to a member of their care team or navigator using a 24/7 helpline.

Training

On-demand access to comprehensive educational classes, content, and support groups on a wide array of topics related to dementia caregiving.

Medication Management

The clinical team regularly reviews prescribed medication as needed, while care navigators provide tips to successfully adhere to medication regimens.

Eligible Patients

  • Have a dementia diagnosis
  • Have Medicare as their primary payer
  • Enrolled in Medicare parts A&B (not Medicare Advantage or PACE)

  • Not enrolled in Medicare hospice benefits

  • Does not reside in a long-term nursing home (assisted living facilities accepted)
