Senior Doc offers all our Medicare patients and members our own personal and customizable Chronic Care Management Program. This is a Covered Medicare Benefit and can be very valuable and instrumental in ensuring the quality of care, follow-up, and outcomes desired for you and your loved ones.
Program includes:
• Monthly check-in Phone call from a Senior Doc Care Manager
• Monthly Medication Review, Reconciliation
• Review and Management of Chronic Conditions
• Overall Care Plan Development, Review, and Modifications
Senior Doc provides medical care to seniors directly in their homes, assisted living, skilled nursing communities, hospice, or via secure Telehealth options.
